Program used: the GIMP
All I used was the paintbrush with a round brush (with 'opacity' and 'size' checked), the ink tool and lots of smudge

edit: as

pointed out, the hand of the girl is.. wrong

so I fixed it in the final version

It's great but there is one mistake. Her right hand is twisted completely wrong. Her thumb should be on the outer side and her little finger closer to the neck.
